1)  Student must be enrolled in a post-secondary program in the field of theatrical arts in Canada. Please indicate school and program title.

Required Information:

      1)    Academic achievement: grade average ___________

2)    State your involvement with school theatre productions if any (i.e. acting, stage crew, costumes, props, lighting, set design/construction, etc.)

3)   State your involvement with local theatre groups if any (i.e. acting, stage crew, costumes, props, lighting, set design/construction, etc.)

4)    State your financial need and future plans involving theatrical arts.
